What is premature ejaculation?

Premature ejaculation is a condition in which a man ejaculates with minimal stimulation. Men experiencing this condition ejaculate before or shortly after penetration.

Premature ejaculation is common. 1 in 3 men report having experienced premature ejaculation at some point in their lives. However, for it to be medically diagnosed as premature, ejaculation must consistently occur within 2 minutes or less from the onset of arousal. The man feels unable to delay ejaculation (uncontrolled ejaculation). Being regularly a source of stress, frustration, and avoidance of intimacy, for some men, a conversation with a doctor may help ease concerns about premature ejaculation. For example, it can be reassuring to hear that occasional premature ejaculation is normal and that the average time from the start of intimacy to ejaculation is about 5–10 minutes.

Symptoms

The primary symptom of premature ejaculation is the inability to delay ejaculation for more than 2 minutes after penetration. However, this issue can occur in all situations, including during masturbation.

Premature ejaculation can be classified as follows:

Primary (premature ejaculation has been present since the first sexual experiences). Secondary (acquired) – premature ejaculation develops after having previous sexual experiences without ejaculation problems. Many men feel they have symptoms of premature ejaculation, but these symptoms do not meet the diagnostic criteria. Instead, these men may have natural variable premature ejaculation, which includes periods of rapid ejaculation as well as periods of normal ejaculation.

Causes

It is not yet clear what exactly causes premature ejaculation. However, experts agree that healthy intimate function requires a combination of physical, emotional, and psychological factors working together in synchronized harmony. If any of these factors are "off balance," premature ejaculation may occur.

Physical factors:
  • Condition (inflammation/infection of the prostate or urethra)
  • Other dysfunctions (erectile dysfunction)
  • Hormonal issues (low male hormone levels)
  • Alcohol abuse
Emotional factors:
  • Poor communication with your partner
  • Relationship conflicts, resentment toward your partner
  • Lack of connection with your partner
Psychological factors:
  • Stress
  • Worrying about intimate performance / Guilt
  • Past trauma: sexual assault, physical or emotional abuse
  • Negative self-image, embarrassment
  • Cultural/religious beliefs related to intimacy
  • Mental health conditions (such as depression and anxiety)
