What is Chronic Prostatitis?

Prostatitis is the inflammation of the prostate gland, located beneath the bladder in men. When it develops, the patient may notice changes in urination patterns. This condition can be serious because the prostate gland produces a key component of semen, which helps transport sperm cells.
Prostatitis can affect men of any age, with the highest risk occurring in the 30 to 50-year-old age group.
In most cases, prostatitis is initially caused by bacterial infections, with symptoms typical of an infection, such as fever, chills, and painful urination, sometimes even accompanied by bleeding. In these cases, antibiotics are the most effective solution to treat and alleviate symptoms.
There are several types of prostatitis, classified based on their causes:
Acute bacterial prostatitis
Chronic bacterial prostatitis
Asymptomatic inflammatory prostatitis
Each type has different methods of diagnosis and treatment.
One little-known fact is that prostatitis can last for several months. In some cases, it may be asymptomatic and only detected accidentally during specialist consultations or tests for other conditions.
Prostatitis can be recurrent, reappearing at regular intervals over time.
Prostatitis can affect any man, regardless of age. However, certain individuals are more predisposed to developing this condition, including cyclists, horseback riders, and those who engage in unprotected sexual activity.
There are no studies confirming the exact incidence of prostatitis among men at this time. However, what is important to remember is that this condition must be diagnosed and treated as soon as symptoms appear. If left untreated, prostatitis can lead to serious complications, such as infertility and acute urinary retention.

Regardless of age, ESWT therapy can significantly improve performance, even in the absence of a specific condition.
Symptoms
Since prostatitis is caused by a bacterial infection, it often presents symptoms typical of an infection, such as fever, chills, and painful urination. The patient may also experience groin pain and a burning sensation when urinating. These are the most easily recognizable symptoms of prostatitis.
Depending on the type of prostatitis, the symptoms may vary from patient to patient.
Symptoms of Acute Bacterial Prostatitis:
This form of prostatitis starts suddenly, with fever, muscle and joint pain, and severe fatigue. The prostate becomes enlarged, leading to a weaker urine flow and increased pain during urination.
Additional symptoms may include:
Blood in the urine
Pain in the penis and lower back
Severe urinary discomfort
If left untreated, symptoms can worsen, potentially causing urinary retention (inability to urinate) and requiring emergency hospitalization.
Other symptoms that can help identify this form of prostatitis include:
- Pain in the perineum (the area between the scrotum and anus)
- Nausea and vomiting
- Burning sensation during urination
- Chills
- Reduced urinary stream pressure
- Penile pain
Causes
Diagnosing prostatitis is not as simple as diagnosing other conditions because its symptoms can easily be mistaken for a urinary tract infection. A specialist consultation alone is not enough, which is why additional tests are always recommended for an accurate diagnosis.
Urine tests
These tests are necessary to help the specialist determine whether a urinary infection is present or not. Sometimes, uroflowmetry (urine flow measurement) is also required. This investigation measures the pressure and flow of urine, helping to identify any obstructions in the prostate, urethra, or pelvic muscles.
Blood tests
They may be necessary for an accurate diagnosis of prostatitis. Blood tests can also reveal signs of a possible infection or prostate-related issues.
Imaging investigations
Ultrasound examination is highly useful in diagnosing prostatitis, providing high-accuracy images (including possible prostatic calcifications) that can assist the specialist. A prostate MRI scan can also be helpful in the diagnostic process.
If not diagnosed in time, prostatitis can lead to acute urinary retention, infertility, and severe dysfunctions of the reproductive system.
